• Student(s) must be present with the parent/guardian at the time of registration
• All Information MUST BE PRINTED CLEARLY in INK
• ALL FORMS MUST BE completely filled out and signed before registration and appointment.
• ANY MISSING DOCUMENTS WILL RESULT IN THE NEED TO RESCHEDULE YOUR APPOINTMENT
